Michael Forman, M.D

Dr. Forman was born in Quantico, Virginia and raised in Fort Lauderdale, Florida. His undergraduate degree in Microbiology and Environmental Studies was conferred with High Honors by the University of Florida. He received his medical degree from The University of Miami School of Medicine, serving as Chief Resident in the Department of Anesthesiology and Pain Management. He is a long standing member of the American Academy of Pain Medicine, American Society of Interventional Pain Physicians, Texas Pain Society, and the Texas Medical Association. He is Board Certified in both Anesthesiology and Pain Management, and is a clinical assistant professor with the University of Texas Health Science Center San Antonio Regional Academic Health Center.

Dr. Forman has been in practice in South Texas for over 20 years previously serving as Chief of Anesthesia services at Valley Baptist Medical Center-Harlingen. Dr. Forman has staff privileges at multiple local hospitals.

Dr. Forman believes strongly in personally spending time with his patients educating them to facilitate maximally the management of their painful conditions. He enjoys the challenges of safely managing complicated chronic pain syndromes utilizing the latest evidence based therapies.

In his free time, Dr. Forman enjoys golfing, fishing, and bird hunting. He is an avid Gator fan. He is married to his wife of thirty years, Tracia. Together they have three wonderful and accomplished adult children.

Dr. Victor A. Pallares is a graduate of the University of Miami Medical School, he completed his specialty training at the University of Miami and Jackson Memorial Hospital Medical Center in Miami, Florida.He is Board Certified in both Anesthesiology and Pain Management by the American Board of Medical Specialties.Dr. Pallares has been in private practice for over 20 years, is a member of ASIPP (American Society of Interventional Pain Physicians), and has been presented The Brownsville Herald’s prestigious Reader’s Choice Award in Pain Management for 2009. He serves on the faculty of the University of Texas Health Science Center San Antonio’s Regional Academic Center.He is Board Certified in Anesthesiology by the American Board of Anesthesiology. Dr. Pallares has a Certificate of Special Qualifications in Pain Medicine granted by the American Board of Anesthesiology.
